HOW TO SUBMIT AN APPLICATION FOR TMI 2013
To submit an application to attend TMI 2013, please fill out the following form:

Deadline: All applications must be received by July 12, 2013.
Fee: $290.00
CAMP FORMS
After your application is submitted and approved, please fill out the following two applications. BOTH of the following two forms must be completed and brought to TMI.
- Permission/Waiver Form
- *Health History Form
*Note: if delegate attended another Salvation Army Camp in 2013, a copy of that camp physical form may be substituted for the Health History Form.
